Family Medicine Provider / Founder
Joe is a board-certified Physician Assistant with a passion for relationship-centered primary care. He believes the best medicine begins by truly listening, taking the time to understand each patient's story, and partnering with families to achieve lifelong health.
Foundation Family Medicine was created to offer a different healthcare experience, one where patients have direct access to their provider, appointments are never rushed, and care is delivered in the setting that best meets each family's needs, whether that's at home, virtually, or in the office.
Joe is honored to care for patients of all ages and is committed to providing thoughtful, evidence-based medicine with compassion, honesty, and respect.
Keleigh Kreilkamp
Co-Founder & Practice Administrator
Keleigh serves as the Practice Administrator and oversees the day-to-day operations that help Foundation Family Medicine deliver an exceptional patient experience. From welcoming new members and coordinating communication to managing business operations and ensuring every detail runs smoothly, she is dedicated to creating a practice where patients feel cared for from their very first interaction.
Together with Joe, Keleigh shares the vision of bringing healthcare back to what matters most—relationships, accessibility, and treating every patient like family.
Outside of the practice, Joe and Keleigh are raising their two boys and are passionate about faith, family, and serving the community they call home. Those same values guide everything they do at Foundation Family Medicine.
